The Preschool Registration Forms can be accessed/downloaded by clicking HERE or by visiting the main office at East Street Elementary School located at 508 East Street between the hours of 9:00am and 2:00pm to pick up the forms.
Please return the forms when completed, making sure to complete all of the required information, as incomplete applications will cause a delay in processing your child's registration.
Enrollment in our program is first come, first served; meaning once a child has been screened, the main office has all registration paperwork and there is an opening, the child's name will be placed on a classroom roster. If there are no current openings the child's name will be placed on a wait list for an available opening. Parents will be contacted when an opening is available.

As part of the Ludlow Public Schools registration process you will need the following information to be available with your completed registration packet.
long form birth certificate that lists parents names
immunization records and current physical exam
most recent report card
court/legal guardianship/ custody documents (if applicable)
The School Committee has adopted the following age requirement:
The Ludlow Integrated Preschool has an open enrollment policy for students who are 3 years of age. Enrollment is contingent upon openings.
Once the registration packet is returned and residency is proven, all special education records and a copy of the most recent evaluations, signed Individual Education Plan (IEP-if applicable) must be brought to the Special Education Department located at 205 Fuller Street.

Parents are responsible for transporting their child to and from the preschool program daily. IEP Teams determine whether transportation services are required for students with special needs.
